Annual Salary: $170,500 Location: Remote ⏳ Job Type: Full-Time As a Telehealth Diabetes Nurse Educator, you will be instrumental in delivering quality patient care through evidence-based education and counseling to individuals with diabetes. This position is ideal for a proactive, patient-centric professional who excels in remote healthcare delivery and values a collaborative, interdisciplinary approach. Key Responsibilities Patient Education & Support Conduct virtual one-on-one and group education sessions for patients with diabetes, focusing on self-management and lifestyle modifications. Guide patients through the fundamentals of diabetes management, including glucose monitoring, insulin administration, medication adherence, and dietary choices. Provide psychosocial support to patients, addressing emotional and behavioral challenges related to diabetes. Customize educational materials to accommodate various learning styles, literacy levels, and cultural backgrounds. Foster a patient-centered approach, ensuring individuals feel empowered to make informed health decisions. Care Coordination & Collaboration Work closely with primary care physicians, endocrinologists, dietitians, and behavioral health specialists to create comprehensive, individualized care plans. Act as a bridge between patients and their healthcare teams, ensuring effective communication and continuity of care. Monitor patient progress and provide ongoing feedback to healthcare teams for treatment optimization. Collaborate with insurance providers and social service agencies to help patients access necessary resources and support programs. Professional Development & Leadership Keep up-to-date with the latest advancements, studies, and recommended approaches in diabetes management and telehealth. Participate in professional development programs, webinars, and certifications to maintain expertise in the field. Mentor and train junior nursing staff, sharing best patient engagement and education practices. Assist in designing training resources and educational initiatives for healthcare professionals. Lead initiatives to improve patient outcomes through data-driven strategies and continuous quality improvement. Technology & Innovation Utilize telehealth platforms, electronic health records (EHRs), and digital communication tools to provide seamless virtual care. Monitor advancements in telehealth technology and integrate innovative solutions to enhance patient engagement and education. Guide wearable glucose monitoring devices and digital health applications that assist diabetes self-management. Ensure compliance with HIPAA and other regulatory requirements regarding virtual patient care and data security. Qualifications & Skills Required ✅ Active Registered Nurse (RN) license in the state of practice. Possess a recognized diabetes education certification such as CDCES or an equivalent credential. Minimum of 3 years of experience in diabetes education, patient counseling, or related roles. Strong knowledge of evidence-based diabetes management, including medication, lifestyle changes, and psychosocial support. Proficiency in telehealth platforms, remote monitoring tools, and digital patient engagement technologies. Strong verbal and written communication abilities, enabling efficient educate diverse patient populations. Commitment to patient-centered care, empathy, and active listening skills.
