Registered Practical Nurse (RPN) — Clinical Care Manager
Ember Care is a new private-pay home care agency serving Kitchener, Waterloo, Cambridge and the surrounding townships. We help people of every age live well in their own homes, through companionship and personal care delivered to a written, individual plan.
We are looking for a Registered Practical Nurse to be the clinical mind behind those plans — the person who meets each new client in their own home, works out what they actually need, writes the plan our caregivers follow, and goes back to check that the care being delivered is safe and matches it.
What you would do
- Assess new clients in their own homes. Function and mobility, transfer status, cognition, falls risk, home safety, and how much the family is carrying. You see what a referral form never shows.
- Write the two documents that matter — the person-centered client profile and the care and activity plan. Goals in the person's own words, interventions a caregiver can actually follow, and the plain-language escalation thresholds that tell them when to stop and call.
- Deliver the clinical part of caregiver orientation — including teaching and demonstrating safe moving and handling. You are qualified to correct technique, which is exactly why we need you.
- Validate caregiver competency in the field — observed visits in the first weeks, a sign-off at three months, and an annual re-check.
- Review and re-write plans as people change.
You do not deliver personal care, administer medication (our caregivers give reminders only — never doses), perform wound care, or carry out any controlled act in a client's home. You do not carry an on-call phone, cover shifts, or hold a caseload of your own. Where a client needs actual nursing, your job is to recognize it and route it to their physician or Ontario Health atHome — never to fill the gap yourself.
Why an RPN might want this
- You use your judgment. The work is assessment, planning and teaching.
- You are not the last line. A consulting Registered Nurse independently reviews every profile and plan you write before care begins. Complex or unstable clients get escalated rather than absorbed.
- Real autonomy on a small scale. You will see the effect of a plan you wrote, because you go back and watch it being delivered.
- Genuinely part-time. Daytime, arranged around you. No nights, no weekends, no rotation.
- Proper employment — T4, WSIB coverage, vacation pay, paid travel time between visits and mileage at the CRA rate. Never a contractor arrangement.
Who we're looking for
- Current registration as a Registered Practical Nurse with the College of Nurses of Ontario — General class, good standing, no restrictions
- Your own professional liability protection (WeRPN, CNPS or equivalent), or willingness to obtain it before you start
- Clear, plain-language written English — your plans are read by people without clinical training
- Confidence teaching and correcting practical technique in front of a small group
- Comfort working alone in people's homes, and the ability to set a boundary kindly
- A valid Ontario driver's licence, a reliable vehicle, and business-use coverage on your own policy
- Current Standard First Aid and CPR-C
- Willingness to complete a Vulnerable Sector Check before any client contact
- Community, home care, geriatric or care-coordination experience
